Patient continues to experience vivid dreams of a waking world where his son survived the car accident. Patient recognizes that this belief is driving a wedge into his marriage because his wife is dealing with the loss alone. His inability to discern between his fantasies and reality could pose a serious threat to his capacity to function on the force. While I do not believe the patient is a threat to himself or others yet, a continuance toward denial will likely result in a downward spiral that could prove catastrophic. His fractured psyche is a symptom of a deep disturbance that cannot be addressed until he is willing to accept the truth about the night of the accident.

Under normal circumstances, confidentiality is a prime condition in establishing an effective therapeutic relationship. Without confidentiality there can be no trust; without trust there can be no therapy.
